Back in High School, our Algebra 2 teacher did the whole Cornell notes thing, and would take points off if we didn't set it up with that format. Do you guys use them?

I've worked with many teachers who strongly recommended a specific problem solving format.

If students worked the problem completely and correctly without following the format, they would get all the points, but partial credit would be closely tied to using the specific problem solving format.
