At Cambridge, a PhD in the department of physics do not need a master degree. But a PhD in the department of applied math and theoretical physics need to go through part iii math first, with very few exceptions.

Personally, I think top 6 in the US > ETH > other colleges. I think doing a master in ETH make you better prepared for a phd, and it's not hard to get into a top US college after doing the ETH master.

It seems you have to apply for the scholarships separately. I have heard that most part iii students have to pay the money from their own pockets. The scholarships are hard to get
If possible, double major in physics and biology + research experience in a biophy group may be the best.
I know an undergrad who major in physics, and did his research in a biophysics group. It seems there is no problem for him to get a top 20 grad school. Of course, he has good GPA and so on.
